Introductory Diving – Medical Declaration
At your arrival you will be ask to fill a medical declaration form. If you think you will tick yes to any of the medical questions you may wish to get a medical certificate from your doctor stating you are fit to dive as to avoid disappointment of being turn away on the day. Please bring the medical certificate with you on the day you want to dive.

Open Water and Advance Open Water Courses – Application Form
Any student need to fill this form and sign a medical declaration. If the student is a minor, his legal guardian will need to sign on his behalf.
